Start-stop method: This maneuver helps to determine the point of arousal. The penis is stimulated till it is about to reach orgasm, and then stimulation is suddenly stopped. Practicing this technique will help to understand the sensations while reaching orgasm. Regular exercising this technique will help you control ejaculation.

Squeeze Technique: Here, the penis is stimulated before orgasm, and gentle pressure is applied on the head and base of the penis. With the little squeeze will, the erection partially subsides, and the penis is stimulated again. This method is reiterated several times. It will help to manage control over ejaculation.

Pelvic floor exercise- These exercises strengthen your pelvic floor and control the sphincter muscle for the urethra. This sphincter acts as a suction-ejection pump that helps to control the ejaculatory reflex. It is a novel approach for treating premature ejaculation that can provide promising results when used with other techniques.

What else to try- Talk openly to your partner. Try to find out solutions together. Co-operation from the partner’s side has shown better results in cases of Premature ejaculation. Psychotherapy: Psychological counseling can help you with stress and anxiety. Couple counseling can help to resolve interpersonal differences that are causing PE and other sexual dysfunction. Medical Management

SSRI- These anti-depressants help to exert the strongest ejaculation delay. These prescription-based drugs are Fluoxetine, Paroxetine, Sertraline, etc. Ejaculation delay starts occurring around the 5-10th day of the treatment. A no reaction period is seen with the regular intake where the clinician needs to increase the dosage to achieve a similar result. This increased in drug dose increases its side-effect profile up to 5 folds. Their usage is limited nowadays. The newer SSRI is rapidly acting Dapoxetine that offers an on-demand treatment.

Topical ointments- These ointments or creams reduce the sensitivity of the penile area. The popular combination is EMLA (eutectic mixture of lidocaine and prilocaine). They are available in gel and spray forms as well. The trans-vaginal numbness is the noted side-effect of their applications.

PDE5i- Sildenafil, vardenafil, tadalafil have proven their efficacy in the cases of Erectile Dysfunction. They have shown the desired effect in the cases of Premature ejaculation as well. However, they are not the first line of drugs for treating PE. In a comorbid state of PE and ED, SSRI and PDE-5 inhibitors are used to improve erectile function and ejaculation.

Surgery- It is the last resort to treat PE where a nerve is being removed, and the penile gets partially hypersensitized. Another technique is the GPA- Glans Penis Augmentation by hyaluronic acid. These fillers are inserted in the glans penis and reduce the tactile sensitivity of the penis that delays the ejaculation time.

Dietary modifications- Diet plays an important role in our well-being. We may not realize it, but a healthy balanced diet regulates all the enzymes in our body. Micronutrients like zinc and magnesium manage important metabolic activities of the body. Food rich in omega 3 and omega 6 manages your lipid profile and increases the ejaculation time.

The definitive treatment for Premature ejaculation is based on the underlying ailment. Stress is bad for sexual and general health. PE does not resolve in a day. Your treatment may take weeks or months to achieve awaited results. Lifelong premature ejaculation cannot be cured but can be managed with above mentioned methods.With these physiological and psychological techniques, up-to 75% men overcame this and have a healthy sexual life.
